I-CARE Home Health Care is grateful to have been providing compassionate, reliable, and trustworthy home care services since 1993. To provide the best quality care, our team is there to assist in every step of our clients’ healing, recovery, and healthy life journey. We emphasize on treating all of our clients and caregiving team members with the utmost professionalism care and respect.
Licensed
I-CARE is a Licensed Home Care Organization in the state of Virginia. It is important to have a home care company that is licensed and surveyed by the state.
Insured
As a home care organization, we hold several types of insurance. All our caregivers, LPNS, and RNs are covered by our insurances.
Bonded
We hold bonding insurance and are humbly grateful that in 28 years of business, have never had a claim.
Why is this important?
Our human resource team recruits some of the best, most responsible caregivers in the area. During the hiring process we inform our caregivers of our expectations and their responsibility of taking care of not only the client but leaving all client personal property intact.
Our experienced administrative team discusses with clients care options, resources, and services that we offer.
Our human resource team goes above and beyond to hire and retain the best caregivers in the area, ensuring they are licensed/certified, fully credentialed and demonstrate our mission daily.
Our Registered Nurses (RNs) work with families to provide a personalized care assessment and implement a plan of care that is appropriate to the client needs. Our nurse aides follow the plan of care as directed, submit daily notes, and report any nuances to the family and our Registered Nurses (RNs).
Our human resource team goes above and beyond to hire and retain the best caregivers in the area, ensuring they are licensed/certified, thoroughly trained, fully credentialed and demonstrate our mission daily.
Every caregiver with I-CARE has successfully passed our Excellence Training Program to ensure the highest quality care. This training program is in addition to the training the caregiver has already received to get their license/certification. The curriculum is created and implemented by our in house nursing and rehabilitation team.
When clients sign up with I-CARE, we immediately start coordinating with our caregiving team. We focus on caregivers that have the experience and skill set to provide the care the client needs. We work diligently to connect the best caregiver for the client and provide regular quality assurance calls/visits to ensure it is a great fit!
Clients are given full details of the caregiver(s) before they arrive at their home and are provided access to our scheduling portal.
Every client with I-CARE receives a personalized In-Home Care Plan. Our nurses (RNs) provide the initial nursing assessment/follow up visits and are diligent about soliciting regular feedback from clients/families to ensure the best quality care.
Care Agreement & Gathering Information
Client reviews and signs our Care Agreement. Once signed, clients receive an e-mail to schedule the nursing assessment.
Care Coordination & Assessment
Client will receive a call from our Clinical Manager to welcome them to I-CARE and answer any questions they may have and get preliminary information from client about their care needs, schedule requirement and characteristics they would like in a caregiver. With the preliminary information given, our administrative team begins the caregiver placement process.
Assessment & Plan Of Care
The RN will provide the nursing assessment and establish a plan of care for the client. This information is sent to the client/family.
Scheduling
With the assessment and plan of care established, the administrative team hones down on the best caregivers that have the experience, availability and skill set for the client’s needs. A team member calls the client/client’s family member to discuss the experience and schedule for the caregiver. Clients also receive the profile of the purposed caregiver with their bio and details of their experience.
Start Care
An I-CARE Team member schedules a time to introduce the caregiver(s) with the client/client’s family. This can be done on the first day of service or a preliminary interview can be scheduled per client request. The client will receive a welcome binder that has daily tasks the caregiver must accomplish. This book is left in the home.
The caregiver that is placed has been give the plan of care and instructions from the client’s assigned nurse case manager and are ready to begin providing services!
Quality Assurance
We love and encourage client feedback! After placement of the caregiver, clients receive regular quality assurance calls, visits and surveys to ensure client satisfaction.
Clients receive quality assurance calls from our RNs where they have the time to express any alterations, they may need to their care plan or caregiver. In addition our management team schedules regular QA visits with our clients to ensure quality care.
Our on-call team is available 24/7 to address and client questions or concerns.
